Our visits include generous one-on-one time with our providers, personalized treatment plans, and in depth education for the following services:
- Integrative Medicine Consultation
- Medical Cannabis Certification, personalized dosing plans and ongoing care (adult and pediatric)
- Education Visit (phone or video for out of state residents)
- Lyme and tick-borne disease treatment
- Ketamine Therapy
- Osteopathic Manipulation
- Naprapathic Manipulation
- Laser Therapy for Pain and Inflammation
Payments are due at the time of visit
- This is crucial for us to offer independent medical care – we work for our patients and not for insurances, pharmaceutical companies, or the government.
- We require payment information be provided at the time of scheduling, and you will be charged after your appointment.
- All visits 90 minutes or longer require a 50% deposit at the time of scheduling. This deposit will be credited to your visit.
- If you miss a confirmed appointment, or cancel less than 48 of our business hours (Mon – Thurs, 9am – 5pm) in advance, this deposit will not be refunded.
- For patients with a past due balance, we require consistent monthly payments with a minimum of 1/6th of their past due balance as well as paying for all future services at the time of scheduling, unless another payment arrangement has been made with our practice manager.
- If you need to be seen but cannot afford to pay for a visit, please contact our practice manager to make a payment plan.

Missed appointments and late cancellations:
- All appointments that are missed or canceled within 48 of our business hours (Mon – Thurs) will be charged 50% of the visit cost.
- After missing or late canceling 3 visits, patients will be required to pay in full at the time of scheduling.
Superbills
- We do not contract with any insurance plan or bill insurance on your behalf.
- We can supply you with a “superbill” or receipt with medical codes that you can submit to your private insurance carrier for reimbursement as an “out of network” provider visit.
- Superbills are not available for Medicare or Mainecare reimbursement.
- Reimbursement depends on your individual plan and we make no guarantee of payment or reimbursement by your insurance carrier.
- Please request a superbill at the time of your appointment.
Requesting Documentation:
- When requesting letters for employers, DMV, or for assistance with outside services we ask for 15 business days to complete them.
- Some forms will require an appointment to complete, such as disability evaluations.
- Medical record requests require a signed and dated HIPAA compliant release of information form.
- Charges may apply for extensive printing and mailing, as well as for medical provider time reviewing records, completing forms, and writing letters.

- We do not have on-call medical providers outside these business hours.

- Call-in hours are limited to 5 minutes per person. If your need requires a longer conversation, please schedule a 15 minute phone visit.

- Please note that provider’s response time can be delayed based on the days they work.
- Urgent medical problems should be directed to your PCP, local walk-in clinic, or emergency room.
- Patient call-in hours:
Prescription refills
- Prescription refills for controlled medications require a visit with your provider at least every 3 months. The length of the visit depends on your personal situation, but may be as short as 15 minutes and be done by phone or video conference. Please schedule these in advance to avoid last minute problems.
- In most cases, 3 months of refills will be prescribed during your visit. If this has not been done for you, please contact your provider through the patient portal or call-in hours, and allow up to 3 business days.
- If you need a change to your prescription, we require that you schedule a visit.
- All other prescription refills can be requested through the patient portal, during call-in hours, or through your pharmacy. Some medications require annual lab work or follow up assessments.
Medical Cannabis Certifications for patients under age 21:
- We require a written referral from the PCP or specialist treating the relevant condition, and permission for ongoing communication/collaboration with that provider.
- We are no longer able to certify patients under age 21 for any psychiatric or mental health conditions, even if we have done so in the past.
