Our visits include generous one-on-one time with our providers, personalized treatment plans, and in depth education for the following services:
- Integrative Medicine Consultation
- Medical Cannabis Certification, personalized dosing plans and ongoing care (adult and pediatric)
- Education Visit (phone or video for out of state residents)
- Lyme and tick-borne disease treatment
- Ketamine Therapy

- Laser Therapy for Pain and Inflammation
Payment
Payment is due at time of service. We accept credit cards, cash, and bitcoin (including lightning). Personal checks accepted by current patients, not for initial visit.
Visits are billed based on provider time spent with the patient, documentation, reviewing records, etc. Initial visits are typically 75 minutes, and follow up visits 15-60 minutes.
Ketamine-assisted therapy sessions take place after an initial visit and are scheduled for minimum 90 minutes with 300min integration follow up visit.

Cancellations
Cancellations less than 24 business hours before and no-show appointments will be charged for the full cost of the visit. This is considerate to our other patients and allows us to spend 15 – 60 minutes in each visit. Thank you for understanding.
All patients will have the opportunity to show proof of an “urgent” reason as to why they were unable to make their scheduled appointment.
No Show
Patients who do not cancel or attend their visit will be charged for that visit, and will need to prepay future appointments. All patients will have the opportunity to show proof of an “urgent” reason as to why they were unable to make their scheduled appointment.
Scheduled Appointments
We understand that delays can happen, however, we must try to keep the other patients and providers on time. We request you come 15 minutes early to your appointment to account for potential traffic and to complete the required intake paperwork. If you are 15 minutes past your scheduled time, your provider may not be able to complete a full visit. If this happens we will do our best to accommodate you and fit you into the schedule later in the day. If you cannot complete your visit you will be charged for the full visit and you will be required to book a new visit.
All initial and most follow up visits require completion of patient intake forms. This increases the efficiency of the visit and the value we are able to provide to our patients.
Account Balances
We will require that patients with an account balance prepay for all future services and set up a committed payment agreement on the past due balances. We reserve the right to require all balances to be paid prior to future services.
Insurance Reimbursement
Although we do not directly bill insurance, many plans will reimburse you for part of your visit cost. We will provide you with a receipt and super-bill which you can submit to your insurance company (not available for Medicare part B). Depending on your insurance plan, you may need a referral from your primary care provider. For more information, call your insurance company and tell them you would like to see an out-of-network provider. Integr8 Health’s providers are opted-out of medicare. Labs and imaging studies ordered by our providers are usually covered by insurance, including medicare, but it is the patient’s responsibility to verify their level of coverage before undergoing the study.
